The Union Medical Group office is conveniently located on the first floor of 2401 Morris Ave., Union. The office is accessible to the handicapped and there is ample parking in the rear of the building.

We see patients by appointment. In the event that urgent care is needed without an appointment, we make every effort to accommodate your needs, while maintaining the appointments of our patients who have scheduled in advance.

FREQUENTLY ASKED QUESTIONS
Question: How do I make an appintment with one of your affiliated specialists?
Answer: You can make an appointment with our staff when you visit or by calling our main number, (908) 688-5000.

Question: How can I get a referral to another doctor? How long does it take?
Answer: In most cases the doctor must examine the patient before in order to provide a refferal. Referrals may take up to three business days, so please plan ahead in making your appointment.

Question:How should I prepare for my physical exam?
Answer:Since a physical exam will require blood testing, do not eat solid foods after midnight before your appointment. Also, if your physical will include an EKG, do not use skin moisturizers or emollients, as these will prevent the EKG pads from sticking to your skin.

Question: Will my insurance plan reimburse me for my annual physical?
Answer: All health plans vary. Be sure to ask your Benefits Manager where you work or consult your insurance company to be certain. In general, you must wait 12 months after your last physical to schedule a new annual physical. Meanwhile, routine office visits are covered.

Question: Do you have information about the new Medicare program?
Answer:The new Medicare program is very detailed. The government provides a Web site where you can find answers to your questions, www.Medicare.gov. You will find a link to this and other healthcare-related sites on our Our Forms and Links page.

PATIENT PRIVACY

All healthcare providers, including Union Medical Group, are required by law to keep our health records confidential.

This law is called HIPAA, which stands for the “Health Insurance Portability and Accountability Act.” It regulates how your health records are shared, stored and used. An associated Privacy Rule requires that all your medical records, including paper and computer files, are maintained in strictest confidence and not shared with anyone unless you give written permission to do so, or under certain circumstances that are required by law.

You have our commitment to respect your privacy and your rights as our valued patient.

PAYMENT POLICY

Payment is expected at the time of visit.

We accept most Insurance Plans. If you have health insurance be sure to bring your card each time you come.

Co-payments, if applicable, are due at the time of visit.

We accept cash payment, as well as Visa and MasterCard. We regret that we are unable to accept checks.

Although we do not accept Medicaid, we do participate in the Family Care HMO.
